Let's start out with really basic issues.  Along the way, I have pointed out that unless there is some compelling reason, a patient's outpatient diabetes regimen should be continued when they are hospitalized.    I will make this easy.  The glucose is 133 on admission.  For this patient, choose from the following and click on your answer:

 

  1. Continue the glyburide, metformin and roziglitazone
  2. Discontinue the oral diabetes medications and start insulin
  3. Continue glyburide, stop metformin, continue roziglitazone
  4. Continue glyburide, continue metformin, stop roziglitazone
  5. Continue glyburide, stop metformin, continue roziglitazone, write an insulin sliding scale
  6. Continue glyburide, continue metformin, stop roziglitazone, write an insulin sliding scale
